WHAT IS A HEALTH CARE HOME TEAM?

A Health Care Home is a team approach where medical and non-medical care is:

- Anticipated before the visit

- Coordinated during and after the visit

- Collaborative with community and specialty services

At the Mankato Clinic, our team of skilled physicians, nurse practitioners, physician assistants, nurses and support staff provide patient/family-centered health care for all of your needs.

 

WHAT IS PATIENT/FAMILY CENTERED HEALTH CARE?

You and your family are experts about your needs and are valued members of the Health Care Home team.  In Patient/Family Centered Health Care, you are involved in your health care decisions.  When patients/families are at the center of the health care team, health outcomes and quality of life is improved.

 

WHAT CAN I DO TO GET THE BEST CARE?

You are at the center of your Health Care Home team, and your involvement is vital in health and non-health related decisions and goals.  The ideas listed below are from patients and families that helped them feel more at ease and effective as partners in their own care:

- Assist in developing a plan of care

- Make goals

- Bring a list of your top questions or concerns to discuss at every visit

- Share communication (such as changes in care plan or lab test results) from all other health care sources with your Health Care Home team

 

WHO IS PART OF THE HEALTH CARE HOME TEAM AND WHAT ARE THEIR ROLES?

You, as the patient and family, are at the center of your Health Care Home team.

Provider (Physician, Nurse Practitioner, and Physician Assistant)
- Directs your care
- Encourages you to advocate for yourself
- Supports your medical decision process

RN Care Coordinator
- Supports communication between you, your provider, school (if applicable), and community services if needed
- Connects you and your family to community resources
- Assists you in creating a plan of care
- Calls you before and after visits to ensure timely, informative, and productive visits
- Provides information and education related to your needs

Primary Nurse
- Knows you as a Health Care Home patient
- Helps answer or direct your questions
- Helps you access appointment times with your care coordinator or provider

Receptionist
- Knows you as a Health Care Home patient
- Helps you get timely visits for the appropriate amount of time to assess your needs
- Communicates messages between you and your team

HOW DO I ACCESS CARE AFTER HOURS OR ON THE WEEKEND?

Urgent Care available in two convenient locations:
1230 East Main Street and 1809 Adams Street
Hours: Monday-Friday  8am-8pm
           Saturday 8am to 5pm
           Sunday 11am to 5pm
No appointment ever needed in Urgent Care.
