2012 Mankato Clinic OB Childbirth Education- Registration Form

Classes will be held at the Mankato Clinic Conference Room @ Madison East **LOCATION CHANGE**. Look for a sign welcoming you to class.
Sibling class will be held at the Mankato Clinic @ Main Street in the OB/GYN lobby located on the second floor.
It is very helpful for us if all registration is received within one week prior to class start date.
We do not send out class verification or reminders, so please keep the schedule as your reminder.
In case of bad weather, listen to any local Mankato radio station for announcements regarding class cancellation.
Class will be canceled if number of registrants are low.
